We have now streamlined leave of absence requests for parents. Any requests must now be made using the form below, the first two pages are to be completed by the parent/carer of the child. Once completed please either return the form to the school office in person, or alternatively this can be emailed to [email protected]. All leave of absence requests must be made 4 weeks prior before the first day of absence. We will review your request and contact you in writing with our decision.


The form is NOT to be used for reporting illness or emergencies, parents/carers should contact the school in the normal way for these.
